7 week old has recurrent ear infections, rash, colic

Hello,

My 7 week old baby girl SCREAMED up to 9 hours/night from the day we brought her home from the hospital. At 1 week, she was given baby Zantac, which seemed to help some.

She also started reducing her bowel movements to 1 every 3 or 4 days, so I took her in and she was diagnosed with a double ear infection at 3 weeks. She was on antibiotics for a week, and upon her return to doctor, one ear was still pink, so we received a new antibiotic.

After 7 days we returned and she still had fluid in her ears, so we were to finish antibiotics. Meanwhile, she developed a scaly, pimply rash on her face/neck. I took her in at 6 weeks for rash (I'll explain THAT later....) and she had a double RED ear infection.. even though we'd continued antibiotics (her last dose had been that morning).

So now she is on heavy-duty antibiotics (and probiotics) now and we have another re-check tomorrow...question about ear infections is:

What in the world causes these infections in a mostly-breast fed baby (at most 1 formula bottle a day, now none)?

The doctor said it could be her catching different viruses, but I'm wondering if it could be allergies and if something can be done at such a young age. She occasionally coughs and sneezes, but has never been very congested and we've never had to wipe her nose or anything.

OK...on to the rash - Dr. suggested at 6-week visit that my daughter had baby eczema was reacting to the formula we were giving her - I'd already been advised to give up ALL dairy, and had done so at 4 days, so we were using Isomil Soy Advanced. Since the 6-week visit she's only received 1 bottle of Alimentum. The rash is considerably improved, but still on face and neck.

We stopped giving her Zantac at 5 weeks as her Upper GI Scan did not show reflux..although the doctor said that she could get reflux when sick. We are to be referred to a GI specialist at our local Children's Hospital, but apparently they won't take us until the ear issues are cleared up, because they could be causing the crying.

My daughter was up until 3:00am two nights last week, had 3 good nights in a row, and then cried for 9 hours yesterday (fortunately ended at 10 pm so we got some sleep).

So my questions are....

1) What could be going on with these ear infections?

2) I've tried eliminating other foods from my diet, still don't eat gassy vegetables, spicy foods, etc. but can't seem to find any pattern between food and crying - is it worth it to eliminate things like wheat? can they do allergy testing this early?

3) Does anything help? Nothing we've tried works... we suspect she has ear troubles plus colic. anyone have experience with this and when it might end? I also have a 2 1/2 yr. old that's difficult to take care of right now.

My goodness! You have been through it! Your story is a testimony of the good mother you are becoming. Although I'm sure this is a difficult time, you are eager to help and serve your baby by finding the answers she needs.

Well done! As a social worker (before the kiddos were born) I sadly must share that not all babies have mothers like yourself. So, kudos for you for enduring those hours of cries (even through tears of your own) and bundling out to the car for visit after visit to the doctor. That screams "GOOD MOTHER".

So...to your questions. I've printed your submission. It's 5:00 and kids are hungry, so I can't answer your submission with the depth I'd like. Therefore...expect ANOTHER comment to your submission sometime tomorrow (probably during naptime!).

Hang in there! You are being the perfect mother she needs right now. Holding and kissing, rocking and cuddling as you, your spouse(?), and your little girl fight through this.

Sorry for the delay, I had some ideas but wanted to run them by my medical expert panel for evaluation first.

Question One: What could be going on with these ear infections?

A possible guess would be to link the infections to the reflux your baby was already diagnosed with. A baby's eustachian tubes are nearly horizontal, making frequent reflux in the back of the throat a possible culprit in the infections. (You can see more about this in this article.)

If you cannot get the ear infection (medical term acute otitis media (AOM)) cleared up within 3 doctor visits, requesting to be referred to an Ear, Nose, and Throat doctor (ENT) would be a good next step.

Question Two: Are there dietary things I should eliminate? Should I do allergy testing this early?

Deciding to eliminate things from you (or your baby's) diet is a discussion you'll need to have with your pediatrician. As for allergy testing, you should ask your doctor about your local Standard of Care for allergy testing on a 7-week-old. Different areas of the country (or world) may test at different times.

The doctor I consulted regarding this issue also offered this conjecture: In the medical community, there is debate about whether there is an association between ear infections and the bacteria Helicobacter pylori which many feel may play a cause in causing peptic ulcer disease. If your infant had H. pylori, that could explain the stomach problems AND the ear infections. That is just a wild guess, but feel free to discuss that option with your doctor for a diagnosis.

Question Three: Does anything help?

Are you referring to the ear infections here? If so, I would request to be referred to an ENT to see if there is a physical reason as to why your little girl's ears aren't draining properly. It may be necessary to place tubes in your baby's ears to help with drainage and healing. (My Elena had to have this done.) You can see more information about tubes in this article.

If you're referring to the stomach issues, it may be a good idea to meet with the ENT at the Children's Hospital before you meet with the GI consultation. That way, the ENT can determine whether it is a physical drainage issue or a reflux issue before you go in to discuss her stomach struggles. That information that would be helpful to the GI doctor in his evaluation.
